(Richland County) - The Occupational Safety and Health Administration is now investigating a fatal construction site accident.
Brian Tingler, 44, of Blythewood was killed Thursday when a piece of construction equipment killed him. It's not yet clear what sequence of events led up to the death.
OSHA will investigate and come back with a report in several weeks.
Investigators say Armstrong Transporation, a construction company out of Columbia, was doing the work at the site. The company said it did not want to comment on the accident.
OSHA says the company has had a total of three infractions in the last six years. In one case, it was for having construction equipment which did not have a back up alarm.
Meanwhile, the other man injured in the accident, Delfino Rabiela, was last listed in fair condition at Palmetto Health.
